I had a whole thing in high school where the office lost my vaccination records and threatened to expel me if I couldn't produce it. (They did not admit they lost it, but it was in 11th grade/3rd year, soooooo.)

They lost mine but told me be I was okay because they had record of one of the vaccines so they believed me on the others
